In effort to keep our patients safe during the COVID-19 pandemic, we are asking that if you meet the following criteria please do not enter the building:

1. Fever of 100.4 or higher

2. Cough

3. Shortness of breath

4. History of Travel to infected areas

5. No children under the age of 18 years.

If you have any one of these symptoms please call 219-762-4999 Option 2 for Dr. Bush. We will be more than happy to help over the phone.
